Teachers, parents and students across Illinois have an opportunity to participate in the Illinois 5Essentials Survey. This survey was designed to generate a detailed picture of the inner workings of your child’s school. As a parent/guardian, this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey about your child’s school.
The Illinois 5Essentials Survey is administered online by UChicago Impact at the University of Chicago on behalf of the Illinois School Board of Education. This survey identifies five indicators that can lead to important student outcomes. The five indicators that impact school success are:
· Effective Leaders
· Collaborative Teachers
· Involved Families
· Supportive Environments
· Ambitious Instruction
School-level reports will be generated if at least 20 percent of parents respond to the survey. Results will be included in the school report card released by ISBE and will also be publicly available via the UChicago Impact website.
Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.
Please complete this survey for your child’s school by following these directions:

- Select the necessary language to complete the survey (English or Spanish)
- Enter requested demographic information
- Advance to survey questions
- Answer all applicable questions
- Click Submit Survey
